CLAIM SUBMISSION REQUIREMENTS
All cash paid claims submissions are made to members@shsbotswana.co.bw
The following documents are essential when submitting a claim;
1.Proof of payment (receipt)
2.Invoice from the attending service provider (with both the doctor & patient signatures)
3.Radiographs ( where applicable)
4.Clinical records / treatment plan (where applicable)
NB: CLAIMS FOLLOW UPS ARE MADE AFTER 30 DAYS
